IMPORTANT NOTICE


WE REGRET TO INFORM ALL PATIENTS THAT FROM 16.3.2020 WE WILL BE WORKING UNDER EMERGENCY MEASURES


THIS MEANS ALL ROUTINE DOCTORS APPOINTMENTS THAT ARE CURRENTLY BOOKED, WILL BE CANCELLED AND GOING FORWARD WE WILL BE OFFERING A TELEPHONE TRIAGE SYSTEM.


RECEPTION WILL NOT BE ABLE TO BOOK ANY DOCTORS APPOINTMENTS


IF YOU NEED MEDICAL ATTENTION, YOU NEED TO TELEPHONE US, YOU WILL THEN BE ADDED TO A TELEPHONE TRIAGE MESSAGE BOOK AND A DOCTOR WILL CALL YOU BACK TO DECIDE HOW BEST TO TREAT YOUR PROBLEM


WE WOULD BE GRATEFUL FOR YOUR SUPPORT DURING THESE TEMPORARY CHANGES AND HOPE TO RESUME A NORMAL SYSTEM AS SOON AS WE ARE ABLE TO

Patient Access

PA

With Patient Access you can book, view, amend and cancel your appointments online via the mobile app and your computer.

You can also view a detailed version of your medical records, including:

  • Demographic details: Your name, address, date of birth.
  • Appointment details: The GP you saw, appointment date, reason for the visit, history, examination and outcome.
  • Allergies and adverse reactions to medication.
  • Medication: For Example, the dose, quantity and when your medication was last issued.
  • Immunisation.
  • Values: For example, blood pressure readings and bloods results.

To register for Patient Access, please visit the surgery and ask one of our receptionists. Alternatively, you can fill out a form online and send it to shoreditchparksurgery@nhs.net with proof of identity attached. This could be either a copy of your Passport or Drivers License.
