IMPORTANT NOTICE


WE REGRET TO INFORM ALL PATIENTS THAT FROM 16.3.2020 WE WILL BE WORKING UNDER EMERGENCY MEASURES


THIS MEANS ALL ROUTINE DOCTORS APPOINTMENTS THAT ARE CURRENTLY BOOKED, WILL BE CANCELLED AND GOING FORWARD WE WILL BE OFFERING A TELEPHONE TRIAGE SYSTEM.


RECEPTION WILL NOT BE ABLE TO BOOK ANY DOCTORS APPOINTMENTS


IF YOU NEED MEDICAL ATTENTION, YOU NEED TO TELEPHONE US, YOU WILL THEN BE ADDED TO A TELEPHONE TRIAGE MESSAGE BOOK AND A DOCTOR WILL CALL YOU BACK TO DECIDE HOW BEST TO TREAT YOUR PROBLEM


WE WOULD BE GRATEFUL FOR YOUR SUPPORT DURING THESE TEMPORARY CHANGES AND HOPE TO RESUME A NORMAL SYSTEM AS SOON AS WE ARE ABLE TO

'Pre-Registration' With The Practice

PreRegistrationIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.You will also need to bring current proof of address, e.g a utility bill, bank statement, tenancy agreement, dated within the last 2 months.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.


Alternatively you may print off a registration form, fill it out and email it to us along with your current proof of address (e.g a utility bill, bank statement, tenancy agreement, dated within the last 2 months).
